Overview

Severe Chronic Lung Disease (CLD) of Infancy or Bronchopulmonary Dysplasia (BPD) is a major complication of premature birth affecting approximately 25 percent of preterm infants with birth weights less than 1,500 grams and 50 percent of infants weighing less than 750 grams at birth. This debilitating lung disease carries high costs in terms of medical utilization and lengthy hospital stays with persistent respiratory illness, pulmonary hypertension, delayed growth and development. In addition to significant morbidities, poor long-term neurodevelopmental outcomes are common.

Management of infants with severe CLD/BPD is extremely challenging due to multisystem involvement and multiple factors influencing the presentation of chronic lung disease. Therefore, a coordinated, evidenced-based, multidisciplinary team approach to the management of infants with severe CLD is crucial to achieving improved outcomes for this high-risk population. Care strategies can be challenging during the NICU stay and frequently these challenges continue throughout the child’s life.

Target Audience

This course is intended for physicians, advanced practice nurses, physician assistants, nurses, respiratory therapists, and the other members of the multidisciplinary team, including occupational therapists, physical therapists, speech-language pathologists, dietitians and social workers who provide care for patients with CLD/BPD. This course will explore the pathophysiology, current research and major clinical aspects of severe CLD management including diagnostic and treatment options. This course will also discuss the long-term needs of this population.

Objectives
Upon completion of this live activity participants should be able to:

  1. Recognize the importance, and components of a multidisciplinary approach to care for infants with severe BPD/CLD
  2. Describe the complexities of care for management of acute and chronic phases for infants with severe BPD/CLD
  3. Examine the evidence-based approaches to care for the preterm infant in an effort to prevent and treat BPD/CLD
  4. Describe the current and future state of ventilatory management of the infant with severe BPD/CLD
  5. Discuss the impact of pulmonary hypertension on the morbidity and mortality of the BPD/CLD patient
  6. Explain the potential application of liquid ventilation in neonates with severe BPD/CLD
  7. Describe trunk pressures (intra-abdominal and intra-thoracic) as critical support for the concurrent needs of breathing and postural stability
