Overview
The care of children with medical complexity largely resides within the medical home in primary care. This course will help primary care providers better recognize and manage the chronic health problems and functional limitations of their patients in a team-based approach. Although children with medical complexity represent a small portion of our patient population, they account for a large number of pediatric visits, resources, and specialty services. This course seeks to help primary care providers improve the quality of care for their patients with medical complexity, while reducing healthcare costs and supporting families.

Objectives
  1. Identify the role of the primary care provider among a large healthcare team in the management of children with medical complexity.
  2. Differentiate roles and improve communication among teams in chronic, complex health conditions, such as cleft lip and/or palate.
  3. Implement management strategies for common challenges in assessing growth or vital signs in patients with limited mobility, tubes, and/or autonomic dysfunction.
  4. Explore genetic causes of medical complexity and the evidence-based diagnostic evaluation approach.

Accreditation

In support of improving patient care, this activity has been planned and implemented by the Children’s Hospital of Philadelphia. The Children’s Hospital of Philadelphia is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
